The whole journey, from capture to result.
Step 01
Log or capture a patient
Add an operation you've done with structured patient, procedure and op-note detail — or skip the typing and photograph a theatre list, OPD sheet or patient sticker to create records in bulk. Any detail can be left blank and completed later.
Step 02
One record, the spine
Everything lands on a single case record: demographics, clinical case-mix (ASA, BMI, comorbidities, Charlson), the operative note, imaging, and a live completeness chip. The patient's other episodes are one click away.
Step 03
Collect the outcome, any door
Press Collect outcome and pick the moment: open it on your device and hand it over, send an email or SMS link, show a QR, print a paper form, or add the patient to a clinic kiosk. The right questionnaires pre-select from the condition.
Step 04
Anyone completes it, encrypted
Surgeon, staff or the patient can fill it in — from any door, any time. On a patient's own device the identity is encrypted client-side; the decryption key stays in the link and never reaches a server.
Step 05
Syncs back & scores itself
Digital submissions are pulled into your self-hosted app and decrypted locally; paper pages are photographed and read by OCR. Each lands on the right case after a quick review, with validated scores already computed.

See risk-adjusted results
Pre-op and follow-up scores line up on a trajectory against the MCID line. Analytics stratify by case-mix (ASA / Charlson / BMI), patients-of-concern alerts fire on decline, and you can Ask plain-English questions over your own data.
